Physician Assistant / Nurse Practitioner

Experience preferred in family medicine. Position will provide direct patient care through patient assessment, follow up examinations, diagnostic assessments, including ordering procedures and monitoring medications.

Clinical Care Coordinator (RN/LVN)

The Care Coordinator is a full-time position focused on engaging patients outside of the patient visit. Successful candidates will work with our physicians and data sources to identify patients in our patient panel who would benefit from coordinated care. Additionally, they will assist patients in the identification of appropriate providers, facilities, and community resources in an effort to improve or maintain the patient’s social, emotional, functional, and physical health status. MAJOR DUTIES AND RESPONSIBILITIES:

  • Coordinates the patient care and delivery of health care services to a select outpatient population.
  • Promotes effective utilization and monitoring of health care resources and assumes a leadership role with the interdisciplinary team to achieve optimal clinical outcomes.
  • Monitors and assures effective utilization of hospital services and resources.

Education and Experience:

  • RN/LVN with active license in the state of Texas or holding a license in the state of their residence if the state is participating in the Nurse Licensure Compact Law preferred.
  • Alternate licensed professionals may be considered, based on experience and overall qualifications as deemed necessary to meet requirements.
  • Prefer 3-5 years experience in direct patient care in a clinical setting, and/or experience specific to program needs along with any applicable certifications.

  • Excellent oral and written communication skills, with problem-solving abilities
  • Previous supervisory experience is required
  • Excellent assessment skills
  • Computer skills including MS Office Suite and spreadsheet manipulation capabilities

Triage Patient Coordinator

The Patient Coordinator will be responsible for handling inbound and outbound communications for multiple Primary Care Medical Practitioners. The Patient Coordinator will be comfortable using multiple communication vehicles and must be able to handle a wide range of both clinical and administrative requests, including but not limited to, scheduling and rescheduling appointments, following-up with patients post-care (primary care visits, specialist referrals and hospitalizations), medication reconciliation and refills, and insurance verification and authorization management.

The Coordinator follows a pre-defined clinical protocol to document complete history of present illness, important patient demographic and insurance information, family, social and medical history, proactively identify special needs, and note any significant care and wellness gaps. The Patient Coordinator also conducts medication reconciliation, documents non-compliance issues and uses the EMR to ensure the right amount of time is scheduled appropriately for each patient appointment.

Responsibilities Include:

  • Update patient medical history, identify needs, schedule appointments
  • Process physician/patient referrals
  • Verify and update insurance information, initiate and complete prior authorizations for care
  • Help patients navigate the healthcare system

Qualifications:

  • Minimum of 1-3 years of experience in a similar profile is preferred.
  • Medical Assistant training
  • Experience working in a medical office and/or call center environment; Primary Care preferred
  • Experience with health care terminology, HIPPA regulations and office procedures
  • Experience with Payor referral and prior authorization requirements and procedures
  • Experience with basic desktop software as well as medical practice management and electronic medical record systems
  • Education: MA Certification, HS Graduate
  • Spanish fluency is a plus

Certified Medical Assistant

Qualified candidates must be certified and have minimum 2 years of recent experience in a medical office as a Medical Assistant.

Responsibilities Include:

  • Obtain medical history & vital signs and record into EMR
  • Perform EKG’s, Injections
  • Triage patient phone calls
  • Answering telephones, scheduling patient appointments
  • Assist physician with procedures
  • Assist in general office duties and other duties as required
